Physical form MUST be signed/stamped by a medical doctor if a Physician Assistant performed the physical.

If student-athlete currently uses ADHD or Asthma medication. Those forms are below MUST be completed.
Sports Physical Form
Must be filled out by physician for student-athletes on a Asthma Medication - The NCAA will drug test for these. There is a letter for your physician with a form to make them permissible.
